Position Summary::Reporting to the Care Advisory Unit Director, the Manager, Clinical Coaching ensures the delivery of high-quality coaching services in compliance with all state, program, and accreditation requirements. Directly managing the Coaching team (clinical and non-clinical coaches), the Manager, Clinical Coaching focuses on the clinical quality care planning and coaching services. In collaboration with key stakeholders, this individual will create action plans within the remote coaching team in support of quality improvement efforts and outcomes. This individual is responsible for coaching staff and developing and delivering training that will enable Care Coaches to execute Seniorlink's Coaching Framework.What You Will Do::

  • Lead and manage a team of clinical and non-clinical care coaches to engage, motivate, and educate family caregivers via phone and technology; may provide direct coaching as needed
  • Provide oversight for the appropriate use of tools and person-centered coaching plans to support caregiver and care recipient needs, e.g. CarePath topics within Seniorlink's care collaboration platform (i.e. Vela)
  • Provide coaching support to team as needed to ensure coaching plans align with clinical outcome goals
  • Provide regular review of critical incidents with Care Teams to identify cause and implication; Recommend interventions, establish action plans and report incidents externally as needed
  • Establish clinical relationships with key external business partners and referral sources to increase care collaboration and foster credibility in our services
  • Analyze care recipient data (e.g. hospitalization rates, falls, incidents, suspension and discharge rates) to determine improvement opportunities
  • Review and support the management of all case activity including activation, suspension, discharges
  • Manage consistency in clinical practice and advise team on policies, documentation requirements, and best practices, ensuring compliance with state/federal regulations and NCQA standards
  • Lead, manage and develop staff to ensure successful achievement of goals
  • Perform other duties as assignedWhat You Will Bring::
    • 7+ years of experience in case management and care planning/assessment within complex care environment; LCSW, RN, or CMC/CCM preferred
    • 5 years of progressive management and leadership experience with proven ability to motivate and develop staff
    • Strong written and verbal communication skills; excellent interpersonal skills with customer focus
    • Tech savvy and comfortable using phone, chat and multiple web-based applications as primary method of communication; comfort with navigating through case management and care collaboration systems
