About the RoleWe are seeking a skilled Registered Nurse to join our team at Mass General Brigham Home Care. As a key member of our interdisciplinary team, you will provide high-quality, clinical services to patients in various settings within the community.Your primary responsibility will be to deliver patient care that meets the highest standards of community health practice, adhering to our organization's philosophy, policy, and standards. You will be responsible for the admissions process and case management, utilizing thorough and timely electronic documentation and interdisciplinary communications.As a skilled nursing professional, you will provide assessment, teaching, treatment, documentation, and care coordination, demonstrating exceptional customer service and financial awareness. Your ability to work effectively with a diverse population, communicate effectively in writing, verbally, and electronically, and work as a member of an interdisciplinary team will be essential to your success in this role.Requirements

• Graduate of an approved School of Nursing
• Current Massachusetts license as Registered Nurse
• Minimum of one year previous nursing experience in an acute care setting, with prior home care experience preferred
• Ability to travel within MGB Home Care's geographic area
• Strong interpersonal and customer service skills
