Bridges is looking for a full-time Experienced Direct Support
Professional for the Day Program in Salisbury, NH ! We are a
non-profit agency founded in 1982 with a mission to advance the
integration, growth, and interdependence of people with
developmental disabilities within their own communities. Building
on the strengths of over 1,100 individuals in the greater Capital
Area and their families, we work to identify barriers of access and
find creative solutions to develop meaningful community engagement.
We are seeking a Compassionate and Experienced Nurse to provide
care for a disabled female person who uses a wheelchair and
communication device. The position is to support one individual who
lives with her family in a private residence in Salisbury, NH . You
will be responsible for helping with daily routines in the home and
community. The person experiences a developmental disability, and
she uses a powered wheelchair for mobility and has a wheelchair van
for transportation. She has medical needs that require constant
attention from a support staff throughout the day. The person
enjoys regular daily routines and activities in their community and
loves learning and seeing new things. The support staff will
provide personal care, medication administration, attend medical
appointments, monitor their health, safety, nutrition, grocery
shopping, community activities, and community connections.
Qualifications: Education: High School Diploma (experienced LNA
preferred) Experience: Experience in the human service or related
field with persons experiencing disabilities is preferred. Reliable
transportation, a valid driver's license, and proof of automobile
insurance) Current Openings: Full-Time: Monday - Friday (40 hours a
week). Applicants must be able to assist in transferring by
utilizing mechanical lifts. Compensation and Benefits: There is a
$1000 sign-on bonus for this position - $500 will be paid at the
time of hire and $500 will be paid after 6 months in the role.
*Please note, sign-on bonuses are for new hires only. Re-hires and
transfers are not eligible at this time. Paid Time Off: For a
full-time employee working 40 hours a week, 22 days of Paid Time
Off is received per year! PTO days will increase over the years
based on longevity and there is no waiting period to start using

Reimbursement: Employees are reimbursed for approved mileage driven
on the clock. Paid Training: As an employee, you are paid your
usual rate of pay while attending training! Specific Duties and
Responsibilities: Assist individuals with planned activities and
community involvement. Develop a relationship map to gather
information about the individual in order to establish social
inclusion opportunities. Assist individuals in developing a network
of people in their lives. Maximize opportunities for the
individuals they support with disabilities to get to know others
and maximize the opportunities for others to get to know the
individual. Assist the individual in obtaining a sense of
self-esteem through active participation in the community. Provide
individualized type and level of support to include, but not be
limited to, personal care and/or skill acquisition etc. Assist
individuals in establishing, maintaining, and presenting a positive
image. This may involve such activities as assisting in the
bathroom, bathing, showering, and dental hygiene, etc. Assist in
developing and individualizing the yearly Person Centered Planning
by providing information for assessments and by developing
implementation strategies for goals/objectives and methods for
monitoring the delivery and quality of support to the individual.
Complete documentation requirements as specified by the Individual
Service Plan to include the scheduled data collection and monthly
progress notes. Assist in the orientation/training of new staff,
which may include introduction to an individual's support persons
and to an individual's ISP, strategies, dreams and goals, and
treatment plan. Distribute medication in accordance with He-m1201
as applicable. Report any safety, health or human rights concern
immediately to the Supervisor. Understand and ensure compliance
with applicable state regulations. Perform other duties and
responsibilities as assigned. Required Skills: Ability to
communicate thoughts and ideas well in both verbal and written
formats in either group or one-to-one settings. Ability to meet
required deadlines for documentation of care. Ability to work
independently without direct supervision and demonstrate sound
judgment and decision-making skills. Ability to assist individuals
to acquire daily functional living skills. Ability to obtain
information about and to utilize community resources, activities
and social opportunities relevant to supported individuals. Ability
to represent Community Choices and its services to the community
with a positive and professional image. Ability to work as a member
of a team and carry out team decisions. Ability to demonstrate
computer competencies: utilize email and word processing
specifically. Ability to demonstrate a positive attitude under
stressful situations with individuals, co-workers and people
external to the organization. Work Environment and Physical
Demands: Work Environment - can be occasionally hectic with
occasional periods of high stress. Must be able to maintain a
positive and professional demeanor in a busy program environment.
Physical Demands - must be physically able to lift up to 50 lbs and
provide transfers as necessary. Walking and standing are required
frequently with occasional stair climbing. Must be able to push
wheelchairs and operate wheelchair lifts/Hoyer and barrier-free
lifts. Emergency behavioral intervention in accordance with MOAB
techniques and individual behavioral plans may be required. Driving
a passenger van and vehicle is required frequently. Travel: Must
have access to a personal vehicle during all scheduled work hours.
Frequently travels with supported individuals utilizing personal
vehicle as well Community Choices vehicles. Must possess and
maintain a valid NH license and safe driver's record in accordance
with Community Choices standards with adequate liability insurance.
