Job Description Job Description We are seeking a Nurse
Practitioner to conduct Evaluation & Management (E&M) visits
for Medicare patients in their homes. This role involves direct
patient care, travel between patient residences, and timely
documentation of visits. Key Requirements: Active, unrestricted NP
license (FNP or Adult NP preferred) Board certification as an NP in
relevant specialty Experience with E&M visits and/or home
health strongly preferred Reliable transportation and valid
driver’s license Strong clinical assessment and documentation
skills Ability to work independently and manage daily schedules
Comfortable using EMR systems for visit documentation DirectShifts
