This is a public health nursing position in a specialized area requiring training beyond what is required to be licensed as a registered professional nurse, or public health nursing work in one of the following areas: (1) performing generalized public health work in an area geographically removed from direct supervision; (2) serving as a team leader; (3) managing a clinic; (4) managing a caseload of patients with acute or chronic medical problems; or (5) serving as a preceptor for nursing trainees or students.

Duties and Responsibilities

The Senior Community Health Nurse is in a highly responsible position with the general emphasis on school health and health education, using skills, knowledge and ability to keep children at their maximum wellness. This position will be directly supervised by the Community Health Nurse Supervisor.

Provide Clinical health care services to students in the School Based Clinics: Including immunizations, family planning, STD/HIV counseling and screening, client assessment (interviewing and counseling), intervention and documentation:; drawing blood specimens and processing those specimens in our labs system. Refer Clients to specialty providers as necessary. Input data into computerized charting systems, monitor system for compliance by Health Aides and LPNs; ability to pull logs, records, and forms from system and accurately compile data for annual reports and School Health Plan.

Monitors and manages health area, providing first aid, assisting with prescription medications for students, takes vital signs and refers abnormal results to parents/nursing supervisor or medical personnel. Administer immunizations for students and adults. This position has permission to transport client's identifiable information in a secure locked bag for work purposes only. Employee must follow all HIPAA guidelines. Employee may travel to other schools to provide services. Employee may be designee in absence of supervisor.

The employee in this position will be responsible for screenings; BMI's (Body Mass Index), vision, hearing and scoliosis, and refer students who perform unsatisfactorily on initial screenings examinations for evaluation. Follow - up on referrals made by teachers and RN's on children who have health problems. Using nursing process to solve problems, and making referrals to appropriate agencies. Ensure that screening date is entered correctly in computerized charting system and HMS.

Record review on all new students. Create care plans on students with chronic health conditions, and students on medications at school.

Provide health education / wellness programs that are approved by school board and principal for students, faculty, and staff.

Attend Teachers / Principal conferences as needed to assist in continuity of care of children. Act as health advisors and consultants in all health matters, ( IEP plan, and 504 ), assist principals, teachers, and counselors and other agencies in family related services. Provide oversight for school health liaisons at assigned schools.

Accurate completion of leave and attendance, Super Bill / HMS forms. 100% of EAR's shall be coded, with 97% completed within a 7 day time span. Assist in extra school activities, i.e.: health fairs, health promotion, etc. Performing all other related duties as directed by the supervisor. Assist with the completion of all school health reports for DOH.

Assist with providing RN coverage and services in the Gadsden County Health Department Clinic as needed.

Minimum Qualifications

  • A bachelor's degree from an accredited college or university with a major in nursing or a related field and one year of professional nursing experience in pediatrics, geriatrics, or public health nursing, and licensure as a Registered Professional Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.27; or
  • One year of experience as a community health nurse with the State of Florida.
  • A master's degree from an accredited college or university in nursing, nursing education, public health, or a related field can substitute for the required professional nursing experience.

  • Conducts a total generalized public health nursing program in an area geographically removed from direct supervision.
  • Organizes, implements, and conducts a specific phase of a large public health nursing program for a county health department.
  • Coordinates, on a regional basis, the provision of health care for developmentally disabled clients in the community.
  • Advises on care and management of illness and refers patients with abnormal findings to pediatricians or other qualified physicians.
  • Assists in planning and conducting orientation programs, continuous in-service educational programs, and professional educational programs for paramedical personnel, auxiliary workers, and volunteers.
  • Demonstrates special nursing procedures and techniques applicable in a variety of medical specialties and teaches the purpose, use, care, and results of utilizing special equipment in the treatment of patients.
  • Interviews and counsels parents on child rearing, growth and development, and nutrition.
  • Performs advanced and extended public health nursing functions in specialized areas such as family planning, maternal health, pediatrics, or geriatrics.
  • Provides public health nursing services, and practices handicapped children's nursing principles in clinics, schools, and homes.
  • Creates and reviews careplans for children with specific medical conditions. Reviews with parents, school employees and professionals.
  • Provides information concerning programs available for children with catastrophic, disabling, and/or terminal illnesses.
  • Performs related work as required.
